I, ___________________________, have hereby retained the services of Waitman Helicopters, Inc. for the purpose of obtaining ground instruction, maintenance instruction, repair or checkout in a Rotorway helicopter. I understand that any helicopter operation entails a high degree of risk and that any operation of an experimental helicopter entails a much higher degree of risk than that of a normal certified helicopter.

I understand that during the course of instruction or repair, it is possible that the aircraft and occupants may sustain damage and injury. This may also result in serious injury or death to the occupants and to those on the ground.

I, therefore, hold harmless Waitman Helicopters, Inc. and its employees, agents and pilots; and accept full responsibility for any damage or injuries from the operation of this aircraft both in the air and/or on the ground, during the course of instruction or repair and during any and all pre or post instruction operations.

I, and my next of kin/heirs, agree under no circumstances will Waitman Helicopters, Inc. and its agents, pilots and employees, be held liable or responsible for any losses (property damage, liability, bodily injury or death) resulting from the operation, maintenance, or storage of any Rotorway aircraft.
